The Physician Assistant Program must ensure that students are free of conditions that might endanger the health and well-being of others in the health care setting. It is equally important that appropriate precautions be taken to protect the health of PA students at clinical sites.
JMU PA students must comply with the screening and immunization requirements of the JMU Health Center, the Virginia Department of Health (VDH) and the Centers for Disease Control (CDC).
Students must provide the Physician Assistant Program with official documentation of compliance with these requirements.
Hepatitis B (Series in process OR Titer)
One of the following is required: Three vaccinations OR positive antibody titer (lab report required)
Waiver is not an option.
Influenza
The following is required: Documentation of a flu shot administered during the current flu season.
Meningococcal Vaccine
One of the following is required: Vaccination record (Initial dose or a booster dose must have been received on or after the 16th birthday.)
Measles, Mumps, Rubella: MMR (2 Vaccines OR 3 Titers)
One of the following is required: Two vaccinations OR positive antibody titer for all three components (lab report required.) Due to the recent cases reported at JMU and many other universities, JMU students and employees are advised to get a third dose.
Poliomyelitis (OPV or IPV): Single Vaccine
Tetanus Diphtheria Pertussis (Td/TDaP):
Required: Documentation of a TDaP vaccine or booster (must be within the past ten years). The renewal date will be set for ten years from the administered date of the booster.
Tuberculosis screening
One of the following is required: Two TB Skin tests (EACH test placed 1-3 weeks apart) OR QuantiFERON Gold Blood Test OR if positive results, provide a clear Chest X-ray.
Varicella (2 Vaccines OR Titer)
One of the following is required: Two vaccinations OR positive antibody titer (lab report required)

Complete the form and ask your health care provider to sign it (required by law).
Log in to your MyJMUChart account to upload a scanned copy of the immunization form
Upload the PDF documents into MyJMUChart – it’s just like attaching a document to an email (DO NOT SEND IT TO THE PA PROGRAM).
Once these records have been verified by the UHC staff, a notice will be posted to MyJMUChart.
Continue checking your JMU dukes email and secure messages on MyJMUChart for confirmation that your record is complete and there is nothing further needed.
Print the immunization log record from MyJMUChart and send it to Marissa Zane at zanemj@jmu.edu. Below is an example of the Immunization Record that needs to be sent to Marissa.
The process for downloading/printing this record has changed a bit since the Health Center has experienced a system update.
Once you get to the screen that lists your verified immunizations, you will need to click on the "back" button on the top left of the list
This will redirect you to a new page that will include a "print" button
Please print the record as a PDF and then email the PDF record to zanemj@jmu.edu
ALL OF THE ABOVE IMMUNIZATIONS MUST SHOW ON YOUR MyJMUChart Immunization Record that you send to the program. If something is missing you must contact the JMU Health Center to get it corrected. Please do not send incomplete or unverified logs to the program.
