STUDENT FEE WAIVER FORM

This form is required annually for ALL students required to pay fees.

• Complete the Waiver form below with information completed applicable to you.

• Return the completed form, prior to the frist event/program, to the school’s  administrative assistant.

• In order to continue participating beyond the first meeting, student(s) must have paid the fee or have been approved for a waiver.
