Documents Needed for Loan Application

You will need to have the following documents available for submission with your application. Make sure to check out samples if you are unsure of what documents we are asking for.

  • 2019 business tax returns. IRS Forms accepted include: 1120, 1120S, 1065, 990 or Schedule C.

  • Most recent bank statement for all operating bank accounts.

  • Accurate 2nd Quarter 2020 Gross Revenue and Net Income - we do not need supporting documents at time of application, but may require evidence of the numbers provided at loan closing.

    1. Gross Revenue is the total of all revenue earned from services or sales of products or any other regularly earned income. This should not include funds received from PPP, EIDL or other COVID related grants.

    2. Net Income is your remaining income after all expenses and cost of goods sold have been paid.

Documents Needed for Grant Application

We want to make this process as easy as possible. We will use the most recent filed occupation tax to determine your grant amount. If you are a single employee business (owner being the only employee), you do not need to submit any documents.

All 501c(3) non profits - Not all nonprofits are required to maintain an active business license. Only nonprofits that are engaged in daily sales of goods and services in competition with for profit businesses are required to maintain an active business license by paying occupation tax each year. We will search the occupation tax database to ensure you have paid two consecutive years of occupation tax.

All for profit Businesses - All businesses are required to maintain an active business license by paying occupation tax each year. We will search the occupation tax database to ensure you have paid two consecutive years of occupation tax.

ANY BUSINESS WITH MORE THAN 1 EMPLOYEE - Any nonprofit or for profit business that has more than 1 person working at the business, other than the owner, is required to provide proof of employee counts. We will accept a variety of documents including GA Department of Labor Quarterly Wage and Tax Report (DOL-4), IRS Form 941, or Partnership agreements (if all employees are member or partners). If you have another form of document, please provide it and we will consider it.
