1. A signed official request letter from the institution/business etc

2. Certificate to commence business 

3. Certificate of incorporation

4. A valid photo ID of the one requesting or senior person in the business

5. list of phone numbers on an excel sheet

6. DOB of requester for instances where Voter’s ID is presented

7. Locational address of business.

One ID card can be used to register a maximum of 15 numbers
