Instructions for Filling Out the COE Submission Form

1. Download the COE Application Form.

2. Please read each section of the form thoroughly before filling it out.

3. All fields must be filled out accurately. Incomplete or inconsistent entries may result in delays or disqualification.

4. Ensure that the entries encoded on the form corresponds to the entries written in the COE Application Form that will also be uploaded in the submission form. Inconsistencies between this Submission Form and the submitted Application Form and the submitted Application Form may cause delays in processing the application.

5. The COE Application Form, along with all required attachments, must be uploaded in PDF format, combined as 1 file. In the submission form, you are required to reference the page numbers in the submitted PDF file to the submitted evidences and narratives written in the Application Form.

6. The uploaded PDF file must follow the following naming convention: [HEI Name][Campus][Program].pdf. 

Example: [Ifugao State University][Lamut Campus][BS Psychology].pdf

Instructions for Filling Out the COE Application Form

2. All fields must be filled out accurately. Incomplete or inconsistent entries may result in delays or disqualification.

3. For sections where a narrative response is required, be concise but specific. Include quantitative data and institutional achievements whenever applicable.

4. The electronic copy of the application form must be submitted via the CHED COE Submission Portal, along with all required attachments, in PDF format, combined as 1 file.

5. Documentary Evidence:

• All required supporting documents must be attached and referenced clearly.

• Each document must be clearly scanned, legible, and submitted as attachment to the application form.

• File names should be properly labeled according to the format:
  [HEI_Name][Campus][Program].pdf
  (Example: [Ifugao State University][Lamut Campus][BS Psychology].pdf)

6. The application form must be signed by the Head of Institution and the Program Dean/Head, including the legal certification at the end of the application.
