EZ-Gap (Signature Benefits)
The Kirksville R-III School District will help employee's pay for their High Deductible through Signature Benefits, administrator of the EZ-Gap plan. The school will pay 50% of claims for a single individual from $1,400-$6,500 deductible portions. After your claim is submitted to Cigna, they will produce and Explanation of Benefits (EOB). Employees will send that to EZ-Merp for cost assistance by using the portal (EZ-Merp will send logins)

SUBMISSION GUIDELINES FOR PRESCRIPTION DRUGS
A copy of a CIGNA Explanation of Benefits (EOB) is always the best documentation.
If no EOB is available, then a copy of the bill from the pharmacy is fine. Ideally this will have the
name of the member, medication and patient payment on it.
