To strengthen the Health, Outreach, Prevention, Education (H.O.P.E.+) initiative model as a
sustainable, interdisciplinary, focused, cross-sectional health access to primary care and mental health services for vulnerable, and marginalized populations in hard-to-reach communities in El Paso County, Texas.

To continue providing access to primary health care and mental health services for vulnerable and marginalized (VM) populations in hard-to reach areas and deliver strategic services, referrals, and follow-ups in collaboration with a network of community organizations and community health workers.
To lay the foundation for a sustainable H.O.P.E.+ Initiative Model in El Paso County, TX. through a network of community engaged collaborators to ensure timely referral, follow up and service coordination.
Organize and deliver six health fairs annually (12 in 2 years) for VM populations, by providing services, referrals, and follow-ups.
Strengthen the collaboration between the El Paso's Federally Qualified Health Centers: Centro San Vicente, Centro de Salud Familiar La Fe, and Project Vida, and expand the network by collaborating with El Paso County’s outreach health centers and non-profit clinics.
Develop awareness and sensitivity of future health professionals to address the needs of VM populations. Support student engagement from the various health professional programs at the University of Texas at El Paso (UTEP) health professional programs (i.e., l Social Work, Nursing, Clinical Laboratory Sciences, Speech, Language and Hearing, and Public Health) and across public universities.
