A brief note of appreciation. Thank you to all of the medical and graduate students whom I have had the sincere pleasure to teach and learn from over the past 40 years. Thank you to all of the academic librarians around the world, whom have worked with me to locate original sources, translate documents, and contributed so much to ensuring the quality and accuracy of information contained in this volume. A special note of appreciation is due Prof. Jacalyn Duffin of Queen’s University. She offered kind, thoughtful, encouragement to pursue my interest in the history of medicine when I was a young Assistant Professor of Medical Neurosciences, many, many, many years ago.
