Pursuant to 4 V.S.A.  27b, you may use this form instead of a notarized statement for documents that would otherwise require notarization. A declaration may not be used for an affidavit in support of a search warrant application, for a nontestimonial identification order, an oath required by 14 V.S.A. 108, or consents and relinquishments in adoption proceedings governed by Title 15A, Adoption Act.

Opinion Release: The Clerk's Office usually releases opinions, if any are ready, at 11 a.m. each Thursday. Court staff posts them to this website as soon as possible. There will be times when opinions are released outside this schedule, such as in emergencies. Scheduled and unscheduled opinion releases are announced via Twitter @flcourts.

Find a national federal court form. National court forms can be used in all federal courts. Each federal court maintains their own local court forms. Use the Federal Court Finder to find a federal court and their local court forms.

Search for national federal court forms by keyword, number, or filter by category. Forms are grouped into the following categories: Attorney, Bankruptcy, Civil, Court Reporter, Criminal, Criminal Justice Act (CJA), Human Resources, Jury, and Other.

Unless specifically noted otherwise, all forms on this page were created by the Alaska Court System. Most forms are fill-in-the-blank, and can be completed either electronically (by downloading the blank PDF and typing in the blank fields), or printed out and completed by hand. Printed copies are also available in person at all court locations. Some forms have the option to use a web-based tool to walk you through and help you complete them. At the end of the walk-through, you will be emailed an attachment of the completed form that you can review for accuracy before filing.

Court forms are not available for every situation. The form books described below often have sample language that you could use to create your own documents to file with the court. But in other situations, you may have to create documents without the help of examples. In that case, you should consider consulting an attorney to assist you. Each person's legal situation is unique. Court staff cannot select forms for you or give you legal advice about what language to use. Court staff can assist you with locating and accessing a form. Court staff may be able to assist you with routine procedures (for example, making sure you have filled out all of the required forms to file for dissolution of a marriage).

The NALS of Anchorage Handbook (formerly the Anchorage Legal Secretaries Handbook) provides explanations of some legal procedures and sample language that may help you prepare documents in the areas of adoption, bankruptcy, divorce, and probate. The NALS of Anchorage Handbook is available for review at the Anchorage, Juneau, and Fairbanks law libraries and at the clerk's offices of the Anchorage, Kenai, and Palmer courts.

The Alaska Court System produces instructional forms and publications, covering a variety of topics. Instructions are available online within their specific topics (see the list at the top of this webpage) and at all court locations statewide. Publications are available online and at all court locations statewide.

A complaint form (available below). In filing a complaint, please identify the nature of the legal matter the lawyer was handling (dissolution of marriage, criminal, personal injury, etc.). If the matter involved a court case, please mention the case name and number. You may attach copies of pleadings, correspondence, or other documents that help to explain the complaint.

In addition, this page connects you to information about lawyers who have been the subject of discipline (opens in new window). On the page you can search by name of attorney, by year or by discipline type. Copies of the applicable court orders and/or opinions are also provided.

Every complaint must be accompanied by the complaint form approved by the Iowa Supreme Court. The blank form may be downloaded here: Attorney Complaint Form (pdf). A fillable PDF form is available here: Fillable Complaint Form (pdf).

Complainants should submit additional documents with the complaint form. If you wish to complain about the conduct of more than one attorney, you must submit individual complaint forms, one for each attorney.

Please do not send CDs, DVDs, or flash drives unless our investigators request those items from you. If you have such items, you should tell us about them in your complaint, and we will follow up if they are needed. Often, our investigators are able to obtain such items in another format, such as from a Court docket.
