Students who wish to sit together at Prom must download and complete this table request form. Only ONE person needs to complete and submit the table request form for the entire group.

You may choose to sit at a table of  10.  

Groups of 10 who wish to sit together will have a guaranteed reserved table only if this table request form is completed. Table choices will only be guaranteed to full groups of 10.

Smaller groups (a group of 3, 4, or 5, for example) should still submit this form. We will do our best to keep smaller groups together, but other guests will likely be seated at your table.

If your name appears on more than one table request form, we will choose your table assignment.

If your name does not appear on a table request form, we will choose your table assignment.

If the table request form includes names of students who did not purchase tickets, those seats will be opened for other students.  

Table forms with any incomplete information will not be guaranteed to sit together.

You must turn in your completed form to the folder outside of room 204 by Tuesday, April 14th.  Students turning in forms after this date will not be guaranteed to sit together.
