The Medications Measure has only 3 questions that your client and their caregiver answer every week. Actually, for clients who are not taking medications, there is only one question. The first questions asks if the client is taking their medications as directed. If they choose “Not prescribed any medications” the questions end there. If they are prescribed medications, they are asked two more questions, one about positive changes and another about any concerns they have about taking medication. The Medications Measure is helpful for clinicians to better understand if the client needs a medication consultation.
