Physical Form & UIL Paperwork

Please complete the physical and UIL online paperwork BEFORE the first day of school. 

Students will be INELIGIBLE to participate until the physical form has been approved and the UIL paperwork has been completed. This includes trying out for volleyball. This is a state mandate. 

Prior to uploading your student athlete's physical form into RankOne, please make sure that the form has been signed by 1) the legal guardian, 2) the student,  and 3) the physician who conducted the physical. Please also ensure that the physical form indicates that the student has been CLEARED or NOT CLEARED for participation. A physical form that does not have the student athlete marked as cleared, and/or is missing one or more signatures, will result in the physical form being DENIED. 

Asthma Form - For athletes that require the use of an inhaler

Please complete the asthma form and submit via RankOne.
