-Children ages 4 to 17 or who are enrolled in a secondary school
-Adults 18+ are eligible for an eye exam and glasses if one of the following can be met:
--->Patients with a valid vision voucher from their primary care provider (CommUnityCare, People’s Community Clinic, Lone Star Circle of Care, Volunteer Health Clinic, Health Alliance for Austin Musicians (HAAM), Vivent Health) (Patients enrolled in MAP or MAP Basic are assigned to one of these partners.)
- The patient is enrolled in Medicaid (specific plans TBA).
- The patient is uninsured AND has a primary care physician at a Federally Qualified Health Center or look-a-line, or at a Nonprofit Health Center such as Hope Clinic, Lirios, Integral Care, Planned Parenthood, Samaritan Health Ministries, etc.
