Required Documents for Final Defense

Final Dissertation Approval Forms

After a successful dissertation defense, THREE forms need to be completed:

1. Candidate Contact Information Form (This form should be completed by all EdD candidates prior to final defense)

2. Final Committee Action Form (This form is to be signed (physically or virtually) by all committee members and submitted to Wendy Turbyfill in the GWU School of Education.)

3. At least three copies of the Signature Page from the candidate's dissertation should be physically signed in black ink by all committee members. Since this page will be part of the printed dissertation, all pages should be on the correct weight paper. (Please see the sample dissertation template for the signature page needed for the printed dissertation.)
