The application must be completed in full. N/A may be used for areas that are not applicable.

After submitting the application please email TXGulfCoastAPHONScholarship@gmail.com the following: 

1. Completed Signature Page

2. Essay

3. Verification of Treatment

4. College/University/Vocational school acceptance letter or report card

**If you are awarded a scholarship we will need your SSN or student ID to submit the scholarship to your school**
