The Required Annual Laboratory Tests and Digital Imaging (DI) for the Faculty and Staff members are the following:
1. Complete Blood Count
2. Urinalysis
3. Stool Examination
4. Comprehensive Blood Panel / Complete Blood Chemistry
5. Chest X-ray
(Note: This will be the requirements for the signing of clearance at the end of the academic year. X-ray result from 6 months before the deadline of submission is considered valid; for the laboratory results, at least one month.)
