How to Apply

Thank you for your interest in applying for the Gray Hair Pet Care program.  We’re glad you’re here, and we look forward to helping however we can. 

As we mentioned on the Home Page, the Gray Hair Pet Care program, offered through Dr. Dennison’s Pet Projects and supported by community donations, helps seniors on fixed incomes access essential veterinary care for their pets during urgent or unexpected situations. The program provides up to 30% off eligible procedures at Plateau Valley Animal Hospital locations in Collbran and Rifle.

Eligibility requirements:

• Must be 65 years or older

• Must demonstrate financial need

• Care must be received at a PVAH facility

• Pet must be a companion animal with an urgent medical condition

• Assistance is available only while funding allows

To apply, please review the detailed program guidelines provided below, then complete an application with personal, financial, and pet medical information.  Please submit the application in person or by email. Applications are typically reviewed within 3–5 business days. If approved, funding is applied (fully or partially) toward eligible services, and care is scheduled or credited to recent visits.
