Entry Form

All artwork submission must include an entry form.

You can either download and print form to submit with hardcopy of artwork OR you can submit entry form online and hand in artwork separately.

1. Please be sure to spell child's names correctly as they will be on certificates.
2. Once you submit this form, you will receive a confirmation email to the email address you have provided.
3. In your confirmation email, we will provide further details on culminating event and celebration information.
4. Please use a new link for each student entry.
