Concerns the "address of the place of permanent residence," applicable solely to Polish citizens and those with registered permanent residence. If you haven't completed this registration, feel free to skip this section.

Point 8

This section can be skipped for your initial zameldowanie, as it requests the address of the previous place of temporary residence.

Point 9

Here, you input the address of your flat.

Point 10

Involves the "Period of intended stay." This period should commence on the day of submitting the documents for zameldowanie and conclude on the last day of your legal stay in the flat. This will be either the final day of your tenancy contract or the expiration date of your current visa/residence permit, whichever occurs first.

Point 11

If you possess a tenancy agreement, there's no obligation to inform your landlord (referred to as the "proxy") about this procedure. Therefore, you can disregard the section with the statement: "I hereby confirm that the aforementioned person remains at the given address," typically signed by the individual with the legal title to the flat. This point is relevant only when individuals lack contracts confirming their right to rent a flat.
