Instructions

Now that you know about Google Forms and Sheets, it is time to practice creating one on your own. You will submit 2 Google Forms (Quiz and Survey) to the class Google Sheet at the bottom of this page. Please use all/any of the following tutorial videos to help you meet the requirements of this task.

For each section, I have created a tutorial video and have also attached a YouTube Video to further your understanding. Please feel free to use any/all resources necessary to complete the assignment.

Requirements for your Google Form QUIZ:

Create a new Google Form and change the theme.
Settings: Collect email addresses.
Settings: Make it a quiz and release grades immediately after submission.
Your Quiz should be at least 5 questions. The first question you add should be "What is your name? (First and Last)".
Create at least one multiple-choice, one checkbox, and one drop down list question on any topic you want.
Make sure you select the correct answers
Add a description to one of the questions (Click on the question and press the "3 vertical dots" button)
Add a 6th Question (Paragraph) at the end of the form saying "What is one thing you liked about my google form? What is one thing I could improve on?"
Do NOT require anyone to sign in: Settings --> Responses --> Requires Sign in: Uncheck any boxes.
Use the "Send" button in the top right corner to copy the shortened link of the Google Form.
Submit the Quiz for other participants to take by pasting the link into Google Form below.
Answer two other participants' Google Form Quiz from your group (same color).

Create a new Google Form and change the theme.
Settings: Collect email addresses.
Settings: Create a confirmation message (ex: Thanks for taking the survey)
Your Survey should be at least 5 questions. The first question you add should be "What is your name? (First and Last)".
Create at least one short answer question, one linear scale question, and one question that contains an image on any topic you want.
Add a 6th Question (Paragraph) at the end of the form saying "What is one thing you liked about my google form? What is one thing I could improve on?"
Do NOT require anyone to sign in: Settings --> Responses --> Requires Sign in: Uncheck any boxes.
Use the "Send" button in the top right corner to copy the shortened link of the Google Form.
Submit the Quiz for other participants to take by pasting the link into Google Form below.
Answer two other participants' Google Form Survey from your group (same color).

Below is the Google Sheet with the data of all the participant responses. The sheet is color coded in groups of 3. Please click on the link of the other 2 participants' QUIZZES, and take the quiz (Don't worry about accuracy, just answer to the best of your knowledge).

*Note if you are the first submission in a group, you will just have to keep checking for other participants to submit.
