All athletes participating in a sport at FVHS must complete an athletic clearance annually.
Below is information on what is needed for FVHS athletic clearance.
All of this info, forms, and links can be found on the FVHS athletics webiste - https://www.fvathletics.com/
1) All athletes must have a current physical to participate.
2) All athletes must have a profile on: www.homecampus.com
3) All athletes must complete ImPact testing

After you have completed all of the above steps, please look for the assigned date that your team will have
there in person Clearance day at FVHS to turn in all of the completed information.
