This waiver is required for participation in our summer program. Complete and bring to your first day of summer practice.

CHSAA PHYSICAL FORM

CHSAA and the State of Colorado have a new physical form that is required as we get our new physicals for the upcoming school year.  It is posted on the Fossil Ridge website and the link is included here. All hospitals/medical facilities/providers are expected to use this form. The athletic department will not be able to accept any other form. All that needs to be turned in is page 4.
