LESSON PLAN FORMATS
There isn't, and there shouldn't be, one 'standard' format for flipped learning lesson plans.
In essence, a lesson plan for Flipped Learning differs from a 'general' lesson plan in that it will indicate the pre-class (individual space) as well.
As for the part about the in-class lesson design, it all depends on the nature of the subject, the lesson objectives, etc. In other words, this part of the lesson plan is not much different from a general lesson plan.
For me, the only difference is that the first part of the in-class design should form a connection between the pre-work and the rest of the lesson.
Note that although the term 'lesson plan' is being used here, flipped learning need not always be conducted as discrete, standalone, flipped lessons. Sometimes a sequence of lessons may go together to form a flipped unit (See Cockrum's examples).

LEARNING PLAN (depends on whether it's a one-period or two-period plan; if the latter, the Flip may come after the first period)
Flip: (eg.., Video defining an Italian sonnet and giving the guidelines for an Italian sonnet.). Sometimes the post-viewing activities may be stated, too.
