Use this MOU if you will be participating in the Field Placements Program through the Audit Track.

You need to complete this form if all or any portion of your work will be done remotely. If 100% of your work will be in-person and on-site, you do not need to file this form with the Director.

You must use this form to document your time each week. File a signed copy to the appropriate line item under "Documents and Forms" each week by Monday at 5 p.m. CST.

Each field placement level will have certain required journal topics. You may use this list to give you ideas for the free writes, or you are free to create topics on your own.

You are responsible for completing this final, 360-degree evaluation form and uploading it to the appropriate line item under "Documents and Forms" in Briefcase prior to submitting and closing out your experience.
