Evolve502 is extremely excited to offer supports during our Community Learning Hub Tutoring Initiative. The tutoring program is designed to support the academic needs of JCPS students from every school across the district in grades K-6 through small group, intentional tutoring.
This site will serve as a "one-stop-shop" for you to easily access presentation materials, answers to frequently asked questions, and more. This site will be updated regularly with more information as it is received. Thank you for your continued commitment to serving our community's youth!

All students must complete and return this form to your organization. Once you have 15 students return completed documents, you can submit them to Evolve502. This is how we will build your roster and create the attendance form.
Using the Google form below, please submit a list of employees and regular volunteers at your organization. This will help us confirm that we have received all necessary background checks.
Click the icon to the right to access the Google form. The MOU is the contract for the Community Learning Hub Tutoring Initiative. The Learning Hub Services Agreement will be emailed to you by Natalie Wynn-Abell after you submit the CLH application and we have met with you regarding your status as a site. All documents will be submitted using the Google form.
Sites will upload the CLH information page, a signed Learning Hub Services Agreement, 501(c)(3), W-9, Insurance, Lease Agreement, and Banking Information. This can be done over time, through multiple submissions on the same form, prior to your start date. You may click "edit your response" to add additional documents after initial submission of the Google form.
