Treatment:
Treatments will only be conducted if 'Energise' deems the treatment safe for the client to receive
GP Consultant/permission may be required before treatments can be provided, if you are unsure as to whether you may require your GP's permission please discuss any concerns prior to booking
Fee payable at time of appointment.
Cancellation policy:
A booking is confirmed once Energise and the client have both agreed a date, time, duration, location for a treatment.
Clients are asked to provide ad much notice as possible when cancelling or re-scheduling appointments.
We have never needed to operate a cancellation charge, so please ensure that give as much notice as possible.
If the client is more than 10 minutes late for an appointment Energise have the right to cancel the treatment.
No additional time will be given for clients late for their appointment, the appointment will end as originally scheduled, and the full fee will be payable.
Energise has the right to cancel a treatment without notice if the room or environment is unsuitable for the treatment to be provided, or if the therapist is taken ill.
Privacy:
Client treatment details and personal information will not be discussed with any third party without prior consent of the client
Consultation forms and treatment notes will be kept on file, securely by Energise in line with GDPR regulations
All clients must complete a consultation form prior to receiving a treatment
By signing the consultation form, clients are giving Energise permission to hold records and data about them
Young persons under the age of 18 must be accompanied by an adult
- The adult must give permission for the treatment to be performed
-The adult must co-sign the client’s consultation form
-The adult must be present in the room throughout the treatment
Insurance:
Energise is fully insured by Balens Insurance
