2. S: Patients verbalize the importance of medication adherence in managing their heart disease.
M: Using patient specific open-ended questions patients will be able to describe how medication adherence leads to improve heart health.
A: Patients will receive targeted patient education on medication regimen, as well at educational materials about their medications.
R: Adherence to medication regimen is necessary for managing heart disease and preventing other health complications.
T: At the end of targeted patient education, patients will be able to verbalize personal medication regimen, and the importance of adherence in relation to their heart disease.
3. S: Patients explain DASH diet for improved cardiovascular health.
M: Using the teach back method patients will explain the DASH diet.
A: Patients will receive target educational counseling and materials about DASH diet.
R: The DASH helps to lower blood pressure in patients with hypertension, helping to improve the cardiovascular health of patients with heart disease.
T: Upon completion of targeted patient education and counseling patients will be able to verbalize how to implement the DASH diet.
