Destigmatizing dementia. By raising awareness, promoting risk reduction, and implementing trainings on dementia for professionals and residents of La Plata County, we strive to reduce the stigma surrounding a dementia diagnosis.


Developing services for those living with dementia. This includes the implementation of dementia care plan(s) in physician office; cultivating a Geriatric Care Center with integrated care; an Adult Day Activity Program; Support Groups, and Social Activities. 


Developing services for care-partners of those living with dementia. This includes support groups, social activities, respite care, care share, networks for support, and grief programs.


Connecting those who have been impacted by a diagnosis with resources that are appropriate for their needs. We accomplish this by maintaining our resource directory and writing early stage resources in purple font so they are easier to discover for those who have been recently diagnosed. 


Crafting an inclusive culture for those with dementia. We seek to create an inclusive culture for those living with dementia by finding and developing resources that are focused on their needs. We firmly believe their experience matters and we seek to amplify their voices. This is why we feature their stories on our website and encourage them to participate in our work and serve on advisory committees that inform local dementia care providers. We feel privileged to have those living with dementia participate in DDC events because we value their insights on how we can better support them and improve our response as a community.
