As phone calls will no longer be answered, we ask that any requests for medical records be emailed to witney.med@gmail.com or sent to us in writing.

Please remember to include your name, date of birth and contact details.

We will contact you and make arrangements to forward your records to your new General Practitioner.

As emails will only be checked intermittently, it may take some time to reply.

We understand that some patients may wish to leave a tribute for Dr Chuen.

In lieu of flowers, we ask that you consider a donation to one of the following organisations:
