We are happy to reschedule your appointment when a conflict occurs. To reschedule or cancel an appointment, please provide at least 48 hours notice. For cancellation requests within 48-hour window, fee will apply, with grace for extenuating circumstances.

Payment is due on the day of your appointment via cash or check.

Please contact your health insurance company to determine whether or not your plan provides coverage and/or reimbursement.  A Letter of Medical Necessity (LMN) or a referral letter may qualify you to receive reimbursement from your health insurance company.
