ATHLETIC PAPERWORK

Starting this year, all athletic paperwork and registration will be online!

The Concussion/Braincourse quiz is required for all athletes. You only need to complete it once during your high school career. https://aiaacademy.org/

ALL PAGES MUST BE FILLED OUT AND SCANNED IN PROPERLY TO THE SCHOOL.  This includes your physical.  

The forms for your physical can be found on DM's website, under Athletics and then under Documents and Links

FEES

There will be two separate fees in order to participate in swim & dive:

1) The AIA sports fee of $200 is required to participate in each sport. This fee is used to help pay for liability insurance, lifeguards, pool usage, etc. This can be paid on Register My Athlete. You will not pay this fee until the coaches have submitted a final roster at the beginning of the season. This will happen by the end of tryout week. You will receive an email when this fee is due. 

2) Desert Mountain swim & dive has a booster fee. This fee is used to purchase the following items for each swimmer and diver: team swim cap, team t-shirt, team sweatshirt, state meet apparel, banquet fee, food and drinks for meets, and end of year gift. The booster fee for the season will be $275.00. 

It can be paid via cash or check made out to DM Boosters. Please drop this off to the president of the booster club, Kristen Yearout, during our parent meeting.

Academic Good Standing

All athletes must be in a good standing with academics in order to participate in any dual meet or invitational.  Grades are routinely pulled, and coaches are notified of athletes who are ineligible to participate in competition.  No exceptions will be made.
