This is a survey I distributed to students at the end of a math unit to collect feedback from them about their learning during that unit. Students were expected to submit the survey anonymously, but could write their name on it if they chose. I found students to be honest with their feedback and reflection. The survey was a good way to reinforce to students that it is about their learning and the importance of them owning their learning. The survey also supported me in my instruction as I could compare the student survey responses with what I perceived and understood about their learning during the unit. It also shows students that we all need feedback to grow and improve. Feel free to download and adjust the survey to meet the needs of your classroom.
