Processing of Barangay Officials Death and Burial Assistance (BODBA)
Certification that the barangay official was incumbent at the time of his/her death using the prescribed template (issued by the DILG C/MLGOO)
Certified true copy of the death certificate of the deceased barangay official
Photocopy of any valid government-issued ID of the claimant
If Claimant is the Widow/Widower: (Certified or True Copy):
Marriage Certificate
If Claimant is the Parent of the deceased Barangay Official : (Certified or True Copy):
Birth Certificate of the deceased barangay official
If Claimant is the Son/Daughter or Brother/Sister of the deceased barangay official:
Birth Certificate of the claimant
If claimant is the Common-Law Spouse/Parent of the deceased barangay official:
The legal heir shall execute an Affidavit or Special Power of Attorney
In the absence of primary and secondary beneficiaries/ claimants:
An affidavit of two (2) interested persons or affidavit of self-adjudication shall be executed by the claimant to support that he/she is the only heir of the deceased barangay official
If a designated beneficiary/legal heir is a minor :
A designated beneficiary/legal heir who is a minor shall be represented by a guardian. An affidavit of Guardianship shall be executed for the purpose
In the case of multiple designated beneficiaries/claimants of deceased barangay officials:
A Special Power of Attorney, which shall be executed as supporting document, except if the claimant is spouse. Only one check shall be issued to the person whom the beneficiaries agreed from among themselves to receive the check.
Other supporting documents from the claimant (whichever is applicable):
Photocopy of Police Report, in case the matter of death of the deceased barangay official is not a natural cause
Affidavit of two (2) disinterested parties shall be executed if there is a discrepancy in the name of the deceased barangay official or claimant
For beneficiaries of IPMRs, if applicable, in the absence of the documents mentioned above, affidavit of two (2) disinterested persons shall be required

Local Chief Executive (LCE) Request-letter
Certificate of Availability of Funds from Local Accountant, or the Head of the Accounting Unit, or in case of his/her absence, the Local Treasurer or Budget Officer ( with the date of issuance, fund source and the appropriated amount), issued 6 months prior to request of vehicle
Certified true copy of an Ordinance approving such purchase (i.e., first two and last pages of the said Ordinance and the page indicating the appropriated amount, per vehicle, to be purchased, and reflecting the name of office the budget was appropriated), and current Sanggunian Resolution on fund utilization in case of trust funds such as, unexpended DRRM fund balances from the previous year(s), and funds transferred/donated to the requesting-LGU from higher-level LGUs, government corporations, etc.;
Updated (current calendar year) inventory/accounting of all existing motor vehicles showing their status/conditions/worthiness (e.g., good, fair, repairable or unserviceable) duly certified by the LGU’s Property/Supply Officer/ Accountable officer. Provided that the same shall not be required if the requesting LGU has already submitted a copy of the same to the concerned DILG office.

Motor Vehicle Types Under DILG’s Approval
Car (sedan or hatchback) with engine displacement not exceeding 2500cc, if gasoline-fed or 3500cc, if diesel-fed, or AFV
Passenger Van and Pick-Up Type Vehicle with engine displacement not exceeding 2500cc, if gasoline-fed, or 3000cc, if diesel-fed, or AFV
Multi-Purpose Vehicle (MPV), with an engine displacement not exceeding 2500cc, if gasoline-fed or 2800cc, if diesel-fed, or AFV
Sports Utility Vehicle (SUV) with an engine displacement not exceeding 2700cc, if gasoline-fed, or 3000cc, if diesel-fed, or AFV**
Van with engine displacement not exceeding 2500 cc if gasoline-fed, or 2800 cc , if diesel-fed, or AFV. A multicab is considered a van.
All-Terrain Vehicle (ATV) with engine displacement not exceeding 800 cc

Basic Documents on All Travel Categories
Request Letter (Note: Governors, Vice Governors, Mayors, and Vice Mayors traveling abroad shall indicate the name of the Highest Ranking Sanggunian Member in their request letter)
A duly notarized affidavit attesting that no administrative charge or criminal case has been filed or is pending against the applicant, or Oath of Undertaking, when the applicant has a pending case;
A duly accomplished Clearance from Money and Property Accountabilities, (CS Form No. 7, Revised 2018) - signed by all Department Heads, indicating if cleared or not cleared and duly signed by the LCE; and
Travel Authorization (or Travel Order or Endorsement) from the Local Chief Executive pursuant to Section 6, OP MC No. 35, Series of 2017

invitation Letter from the host country or sponsoring agency;
Acceptance letter from CHED, TESDA, LGA or organizer, or donor, if applicable;
Copy or draft of the Memorandum of Agreement or Memorandum of Understanding between the LGU concerned and the LGU abroad (travel relating to Sister City or LG Technical Exchange Cooperation), if applicable.

For Governors, Mayors of Highly Urbanized Cities and Independent Component Cities, Mayor of Pateros
(a) Duly accomplished Application for Leave (CS Form No. 6, Revised 2020)
(b) Medical Certificate for medical reasons
Other Elected Officials (including employees - department heads and below) when the period of travel extends to more than three (3) months or during a period of emergency or crisis

Letter Request of Local Chief Executive stating the purpose
Duly approved Three-Year Peace and Order and Public Safety Plan
Local Peace and Order Council Resolution duly approved by two-thirds (2/3) of the total membership approving the release and use of additional CF
Certification from the concerned PNP Chief of the locality relative to the peace and order situation highlighting in concrete details the circumstances which require the urgency in allocating confidential activities
Annual and Supplemental Appropriations Ordinance indicating the appropriations for CF
Certification from the Local Budget Officer as to the availability of appropriations
Statement of itemized POP duly certified by the Local Budget Officer
Certification of compliance with the Full Disclosure Policy posting requirement in at least three (3) conspicuous places for the last 2 quarters, duly signed by the DILG Field Office concerned and noted by the DILG Regional Director and attested by the CSO Representative
Physical and Financial Plan, for both the original allocation for CF and the subject request, indicating the proposed amount allocated for each program, activity and project
Original or certified true copy of Oath of Office (Panunumpa ng Katungkulan)
Certification from the following Officials as to the inclusive dates of services rendered with Satisfactory Performance and shall include the date and place of birth of the Barangay Officials (applicant) using the revised CSC form dated April 2012:

Appointment as Barangay Treasurer/Secretary Resolution duly concurred by Barangay Treasurer/Secretary
Authenticated copy of Birth Certificate and Marriage Contract (if applicable)
Letter Request
Photocopy of a valid ID of the Official
Certification of Services Rendered issued by the barangay (for barangay officials only)
In case of discrepancy of the name, the client must secure any of the following:
Oath of Office (Panunumpa ng Katungkulan)
Joint Affidavit of 2 Disinterested Parties
Photocopy of Birth Certificate/Marriage Certificate
Letter addressed to the DILG Provincial Director requesting a Resource Person.
