STEP 1: Download the Citizen's Charter Service Request Form (CCSRF):
Step 2: Once the CCSRF has been duly accomplished,
please email it with the documentary requirements to 📧 dil9cityfieldoffice@gmail.com, or hand carry the documents to 🏬 DILG-Baguio City Field Office, 2F City Hall Building, Baguio City 2600 to review the documents submitted.
Once submitted, a notification will be received by the client through email/text/call acknowledging the receipt and to provide other updates on the Certification.
