The Challenge Context
The Central California Alliance for Health (CCAH) region—comprising Mariposa, Merced, Monterey, San Benito, and Santa Cruz counties—faces significant healthcare access barriers, especially for underserved populations. Rural residents across these countries struggle to access healthcare in a timely manner due to transportation limitations and a lack of local facilities. This issue disproportionately affects seniors and individuals with chronic health conditions who require frequent medical services. Additionally, members with limited English proficiency, particularly Spanish speakers, face challenges navigating the healthcare system, further exacerbating access issues.
Below are a few example areas of challenges that teams can address, provided by Omatochi:
Transportation Access Solutions: How might we help rural seniors and individuals with chronic conditions access healthcare facilities when transportation is limited?
Care Coordination Technology: How might we create tools to help Care Managers coordinate medical appointments, medications, and provider communications more efficiently?
Language Access and Navigation: How might we help limited English proficiency individuals, particularly Spanish speakers, better navigate the healthcare system?
Social Isolation Prevention: How might we reduce loneliness and social isolation among seniors aging in place?
Family Caregiver Support: How might we provide tools or resources to support overwhelmed family caregivers who are balancing work and caregiving responsibilities?
Nursing Facility Transition Support: How might we support seniors transitioning from nursing facilities back to community living or assisted living facilities?
Medication Management: How might we create solutions to help seniors manage multiple medications and remember to take them on schedule?
Provider Communication:How might we ensure better coordination and information sharing among multiple healthcare providers serving the same patient?
