Consent
• All services require informed consent before treatment begins.
• Clients may withdraw consent at any time during the session.
• Therapists will explain techniques and obtain verbal agreement before proceeding.
Modesty & Comfort
• Proper draping is maintained at all times to ensure client modesty.
• Clients are encouraged to communicate any discomfort immediately.
• Adjustments to pressure, positioning, or techniques will be made promptly to support client comfort.
Information Sharing
• Health history and relevant information must be shared honestly to ensure safe and effective care.
• All client information is kept confidential in accordance with professional standards.
• Therapists may request updates to health information before each session.
Right to Refuse Services
• Services may be declined if a client’s condition is outside the therapist’s scope of practice.
• Therapists reserve the right to refuse or discontinue services if safety, boundaries, or professional standards are compromised.
• Clients also retain the right to refuse any technique or end a session at any time.
A therapeutic session tailored for teens, blending Swedish, Orthopedic, Sports, Trigger Point, and Myofascial Release techniques. Focused pressure is applied mindfully to relieve tension, support recovery, and promote deep relaxation in a safe, nurturing environment.
• Age range: We serve adolescents ages 5–17; for clients under 15, a parent or guardian must be present in the treatment room for the entire appointment, or the appointment will be rescheduled.
• Ages 5–15: A parent or guardian must remain in the room for the entire appointment. If the parent or guardian leaves, the minor must accompany them.
• Ages 15–17: Services may be provided without a parent or guardian present if consent is obtained from all parties. In these cases, the adolescent can request a therapist of the same gender. If unavailable the parent or guardian must accompany and remain in the room.
• Consent: A signed parental consent form is required at first visit.
• Modesty and comfort: Draping is used at all times; therapists will only work on areas agreed to in the intake form.
• Therapist qualifications: All adolescent sessions are provided by therapists trained in pediatric/adolescent bodywork and trauma‑informed care.
Massage Therapy Minor Consent Form will need to be complete at the time of your appointment.
