Eligibility requirements are listed below. To be taken to the online scheduling tool scroll to the bottom of the page and click the appropriate link based on your age and previous vaccination status. You will be able to schedule a flu shot at the same time if you wish to do so.
Eligibility and Recommendations for COVID-19 Vaccinations by Age
Adults 65+:
Everyone over the age of 65 who has had a previous dose of Covid-19 vaccine is indicated for mNEXSPIKE.
Individuals between 12 and 64:
Individuals who have an underlying medical condition (listed here), or who wish to get vaccinated against COVID-19 are eligible for mNEXSPIKE if they have had a previous dose of COVID-19 vaccine. If you have not received any COVID-19 vaccine you are eligible for Spikevax. We have ordered some adult doses of Spikevax but they have not yet arrived. Insurance coverage may also play a role in which vaccine (mNexspike vs. Spikevax) you are able to receive.
Children aged 6 months (5 years for us to vaccinate) to 11 years
The American Academy of Pediatrics published their own vaccination schedule on August 19, 2025. You can find that schedule here. The document is lengthy but mirrors the format of previous ACIP vaccine schedules. To summarize their Covid-19 vaccine recommendations:
Routine vaccination
Children ages 6 months to 24 months should receive the Covid-19 vaccine. All vaccine doses should be from the same manufacturer. (This is for your information only, we cannot vaccinate children this young)
Unvaccinated:
2 doses Moderna at time 0 and 4-8 weeks later
3 doses of Pfizer at time 0, 3-8 weeks, and then at least 8 weeks after dose #2
Incomplete initial vaccination series:
1 previous dose Moderna: complete initial series with 1 dose 4-8 weeks after most recent dose
1 previous dose Pfizer: complete initial series with 2 doses 8 weeks apart (administer dose 1 3-8 weeks after most recent dose)
2 previous doses Pfizer: complete initial series with 1 dose at least 8 weeks after the most recent dose
Completed initial vaccination series:
1 dose of Moderna of Pfizer at least 8 weeks after most recent dose
Special situations
Age 2-18 years in the following risk groups: persons at high risk of severe COVID-19, residents of long-term care facilities or other congregate settings, persons who have never been vaccinated against COVID-19, persons whose household contacts are at high risk for severe COVID-19. This age range is broken up further below
Ages 2-11 years: 1 dose of Spikevax or Pfizer regardless of previous vaccination status at least 8 weeks after most recent dose (we offer vaccines for children beginning at age 5).
Ages 12-18:
1 dose of Moderna Spikevax, Pfizer Comirnaty, or Novavax Nuvaxovid regardless of previous vaccination status at least 8 weeks after the most recent dose
1 dose of Moderna mNEXSPIKE for people who have previously received a COVID-19 vaccine. This dose should be administered at least 12 weeks after the last dose was received.
Of Note: Children 2-18 years of age not included in the above risk groups whose parent or guardian desires their protection from COVID-19 should be offered a single dose of age-appropriate COVID-19 vaccine per the recommendation of the American Academy of Pediatrics.
These vaccines are no longer provided to us for free. We will continue to bill pharmacy insurance providers. However, your insurance may restrict where you can go to get the vaccine and which vaccine you can receive. We recommend that you provide your insurance information either via upload to the scheduling form, or by contacting the pharmacy to ensure coverage before your appointment.
APPOINTMENTS
mNexspike appointments for ages 12+
Appointments will run from 9:00 to 4:00 Monday through Friday. Appointment slots will be released on a two week window with new appointments released on the 2nd and 4th Thursday of the month. If you or your child have previously received a dose of an COVID-19 vaccine it will need to have been at least 3 months since their most recent dose. You will need to provide this information when scheduling your/their appointment.

Doses for children aged 5-11 years old
We can provide COVID-19 vaccine doses to children as young as 5. Appointments will run for varying times Monday through Friday. Appointment slots will be released on a two week window with new appointments released on the 2nd and 4th Thursday of the month. If your child has previously received a dose of an COVID-19 vaccine it will need to have been at least 2 months since their most recent dose. You will need to provide this information when scheduling their appointment.
