During my time with Poway Unified School District, I streamlined the data collection and analysis process by creating various Google Forms to use within a comprehensive school counseling program. I was later invited to present and share these documents with all elementary counselors in the district. View my presentation here.
Staff can complete this Google Form to refer a student to the school counselor. Staff provides information about student, interventions that have already been tried, priority, etc.
Use this Google Form to keep track of student meetings: who, when, topic of meeting, which ASCA domain was addressed, and notes.
Data from this form can be used to monitor student progress and total the number of individual student meetings held throughout the year.
Use this Google Form to record all of those times when students are sent to the office for behavior incidents, with or without a written referral. Keeps track of who, when (date and time of day), type of incident, and consequence.
Data from this form can be used to identify recurring reasons for referrals (and to adjust your core curriculum accordingly) as well as to identify certain times of the day when students may need additional support.
Use this Google Sheet to keep track of each time you or someone on your team delivers a classroom lesson.
Data from this form can be used to total the number of classroom lessons delivered throughout the year.
