VALIDASI PENILAIAN KAUNSELING UBAT HKL

1. All pharmacist are required to be validated at least once a year from their respective units.

2. All PRPK are required to to be validated by their assigned pharmacist from the Outpatient and/or Inpatient Pharmacy.

3. This validation form is consist of 2 sections, which are COMPULSORY (page 1) and OPTIONAL (page 2).

4. All Pharmacist and PRPK are required to complete the COMPULSORY page.

5. For Pharmacist,each head of unit has to ensure that the validation forms that has been completed should be compiled in one file for audit purpose

6. For PRPK, the form should be submitted to Pusat Sumber Farmasi within 2 month of Hospital Attachment.
