Important notes: August-September is combined. Any hours used before a final contract is received will be charged at the drop-in rate. Be sure to clarify the date when you will begin your contract schedule, otherwise all contracts begin on the first day of school.
4.) Ext Care Schedule/Contract Change Form 2016-2017
* For schedule changes throughout the year. Forms must be received by the 25th of each month, effective the 26th. Each contract will continue until you notify the Director of a change or cancellation of the contract. Please keep a dated copy of your change for your records.
* Note: August-September is combined. May-June is also combined.
