MH-Smile was a 12-month Health Way funded program aimed at improving the physical health comes in people with mental health concerns/illnesses, living in the Cockburn regional area of West Australia.
In collaboration with Nurse Practitioners at Cockburn Wellbeing reduced the physical health disparity between this group and the general population. The areas specifically targeted were improved dietary intake and physical activity levels, diabetes, cardiovascular, metabolic and respiratory disease risks, reduce alcohol and other drug use and improved oral health and sexual health.
Clients who met the criteria were screened and monitored for mental and physical health issues over a 12-month period, with four follow-up health assessments.
