The Cotabato Medical Foundation College, Inc. welcomes qualified applicants with good moral character; single or married, regardless of gender and sexual orientation, religious affiliation and ethnic group or nationality.
These regulations apply to all students who are enrolled in this school and who, upon admission, agree to abide by these regulations and conduct themselves in such a way that upholds the good name of the school and actualizes its vision, mission and goals.
This institution respects student’s sexual preferences in their attire such as those belong to the LGBT group.
CREDENTIALS FOR ADMISSION
Freshmen
Students seeking admission must meet the following requirements:
1. Graduated from secondary level in a private or public school recognized by the Department of Education.
2. Passed the entrance examination given by the college.
3. Satisfactorily passed the interview by the Screening Committee.
4. Medical Certificate issued by the School Physician.
5. Submission of the following documents to the Registrar’s Office:
a. Form 137-A or the original report card
b. Certificate of Good Moral Character
c. Birth certificate (photocopy) from Local Civil Registrar and NSO
Marriage Contract (for married applicants)
d. Physical examination result (chest x-ray, urinalysis, fecalysis and CBC result)
e. Two copies of 2x2 ID pictures
f. Two long brown envelops
Transferees
Transferees have to submit the following requirements:
a. Honorable Dismissal
b. Certificate of Good Moral Character
c. Official Transcript of Records (photocopy) from previous institution for evaluation purposes.
d. Birth Certificate from Local Civil Registrar and NSO (photocopy)
e. Marriage Contract (for married applicants)
f. Physical Examination result (chest x-ray, urinalysis, fecalysis and CBC result)
g. Two copies of 2x2 ID pictures
h. Two pieces long brown envelop
EVALUATION OF CREDENTIALS
1. The following are not valid credentials or records for purposes of registration:
a. A transfer card (Form 138-A) which does not state the eligibility of the
students to the course, although signed by the Principal or Registrar; is marked “used for enrolment in another school” or “cancelled”; or has erasure or alterations.
b. A diploma
c. A Transcript of Records marked “For Evaluation Only”, “For Personal Reference Only”, “For Employment Purposes Only”, “For Records Purposes”, “Not Valid for Transfer”, and etc.
d. Form 138 or Transcript of Records of applicant from any private school which has no government permit to operate.
2. The Office of the Registrar shall be the only authorized office to make an official request to the school concerned for an official copy of student’s credentials.
3. Any credential with erasures shall not be honored, unless signed/initialed by the Registrar concerned.
4. The Registrar usually signs credentials or records. If there is doubt as to authenticity of the signature or the signatories, the Office of the Registrar shall take action to verify its authenticity and validity.
5. Transcript of Records of graduated students entering the Graduate School, pursuing a second degree, taking professional education units or taking supplemental subjects shall contain the degree obtained, date of graduation (month, date, year) CHED Special Order Number (for private schools), and Registry Order (for accredited private schools).
TEMPORARY ENROLMENT
The temporary enrolment may be allowed on a case to case basis, subject to the approval of the Dean of College in consultation with the Registrar and the VPAA. A student may present the affidavit of undertaking, promise to submit the necessary requirements or credentials within a specific period to be determined by the Registrar. Failure of the student to comply with the requirements within the period as promised will not entitle him/her to any credit of the subjects enrolled and any payment made therefore shall be forfeited in favor of the school.
ENROLMENT AND REGISTRATION PROCEDURE
The enrolment process is as follows:
A. Freshmen, Transferees, Returning students shall:
1. Proceed to the Guidance Office for the entrance examination, interview, updating of student’s information and completion of other requirements.
2. Fill-up Students Admission Form (SAF), arrange and secure approval for the subject loads from the Dean.
3. Submit loads to be encoded and printed in the Registrar’s Office.
4. Pay the required entrance fee or down payment/ full payment to the Business Office. Freshmen who may qualify for academic scholar may secure application form from the Office of the Dean and provide the following:
a. Photocopy of the Form 137-A
b. Original and photocopy of certification from the high school principal
5. Comply the necessary signatures needed in the Student Admission Form (SAF)
6. The form will be stamped “enrolled” by the Registrar which marks the end of enrolment process.
B. Old Students shall:
1. Proceed to the Registrar’s Office for the evaluation of grades. Secure, fill up Enrolment Form (SAF) and seek approval for the subject loads.
2. Submit subject loads to be encoded and printed in the Registrar’s Office.
3. Pay of the required entrance fee or down payment/full payment to the Business Office. Academic scholars may apply and secure application form from the office of the Dean and provide the following:
a. Computation of grades taken from the previous semester.
b. Signature of the Registrar shall be obtained for confirmation of grades.
4. Comply the necessary signatures in the Student Admission Form (SAF)
5. The form will be stamped “enrolled” by the Registrar which marks the end of enrolment process.
CROSS-ENROLMENT FROM CMFCI TO OTHER SCHOOL
A student may be allowed to cross-enroll in another school under the following conditions:
1. The student is graduating at the end of the academic year.
2. The subject to be cross-enrolled is not a major subject and is not being offered in the CMFCI.
3. The subject to be cross-enrolled belongs to the old curriculum and is not being offered in CMFCI.
4. The maximum number of units allowed for cross-enrolment is six (6), unless otherwise they are already the last number of units to be taken by the student to qualify him/her for graduation.
5. Request for cross-enrolment shall be recommended by the Dean, noted by the Registrar, and approved by the Vice President for Academic Affairs.by the student to the chosen school.
6. The in-charge of records should be provided a copy of this approved request.
CROSS-ENROLMENT FROM OTHER SCHOOL TO CMFCI
Students from other school can be admitted to CMFCI as cross-enrollee on the following conditions:
1. The student should present a study permit/permit to cross enroll at CMFCI.
2. The should pay the corresponding fees such as Miscellaneous fees and the number units of subject intended to enroll.
3. The student should abide with the rule and regulation of the school.
LATE ENROLMENT
If a student enrolls after the start of classes, he/she will be charged P100.00 per day for late registration. The date of his registration shall be the first day of classes according to the College Academic Calendar.
