Log-in or Create an account at the Philhealth Member Portal (https://memberinquiry.philhealth.gov.ph/member/) 

Step 2:

Click ‘Select a Konsulta Provider’ 

Step 3:

Click ‘Search other Konsulta Provider’ 

Step 4:

Choose ‘Cebu’ as Province. Choose the municipality or city with the nearest Clinica Prime branch, then, click ‘Search’

Click ‘Clinica Prime Cebu, Incorporated’

Click ‘Yes’ to confirm
