Note :

Try to do the following before filling out the application and seek help from your pediatric cardiologist or Cardiologist or Internal medcine or the nearest health center to do the following :

The The patient's temperature, pulse, oxygen, Blood pressure , Heart rate ,number of breathing per minute, ECG and a chest x-ray (if possible) .

If the patient  after cardiac surgery for the first time - please do in addition to the above: examine the wound and ensure that there are no secretions or inflammation, blood analysis (CBC, kidney function, electrolytes, liver function).

If the patient is taking blood anticoagulant - please perform (INR) test before filling out the application.

Verify the type and doses of medication the patient is taking.

The patient will be given a regular medical report in English showing the patient's current condition and the proposed treatment plan, which can be used when visiting any medical clinic or other health institution.
