Absence Form
Fill out this form if you plan to be absent for any reason, the coaches do not want valid absences to be a reason you cannot compete in a meet.
Meet Transportation
If you are driving your own swimmer home from a meet (including invitationals & state) you must download and complete the transportation form.(https://drive.google.com/file/d/1Dc3fILRbRu9S0B-z9aYG5fwFe6RuwTMQ/view?usp=sharing)
Note: If you have two or more swimmers on the team you need to complete one form for each swimmer.
Download and print the transportation form.
Fill out the form and make sure to include all meets (see list below or visit the team calendar on the "Schedule" page) you plan to drive your swimmer home from so you only have to turn in one form.
Have your swimmer drop off the form at the H4 Desk.

Payment with InTouch Portal
Each sport has a required participation fee. Once the teams are established, athletes should pay this fee through InTouch. All athletic activity fees for these sports must be paid prior to the first competition. Please go to the CFHS website, click on InTouch Payments, login, click on your student's name, Activities at Student's School, Athletics. You will select the sport that your child is participating in. Refunds will not be issued after two weeks of practices.
