You should receive a SIGNATURE FORM Receipt email from CEASC Support within two weeks after sending in the forms. Contact CEASC Support (AvocadoSingers@gmail.com) if you do not receive the receipt.

It is possible for the paper forms sent in the mail to get lost in transit. If the SIGNATURE FORMs is lost in the mail, you will be asked to send another copy. Be sure to make copies of the completed SIGNATURE FORMs when you first receive them from families.
