The same instructions apply: please provide verbal feedback, complete the form (printed or electronic PDF) within 7 days, and either give to the resident who will upload it into their ePortfolio or send via hospital mail to Lori Dengler at UH Dept of Anesthesia.
Since most other departments are not using the Royal College's ePortfolio system yet, you may print out the PDFs of the relevant EPA assessment forms.
Printed PDFs of assessment forms
- Print out a copy of the EPA assessment form to give to your Observer/Assessor
- Request the EPA and likely explain what it is
- Ensure you receive the completed form—don't forget to ask for feedback before the assessment takes place so that the feedback you receive is constructive and based on an observation!
- Upload your completed form in ePortfolio no later than 7 days after your request
