Prenatal Visit --(125$) 1 to 1.5 hours--A prenatal visit includes a detailed medical history, limited physical exam, a written care plan, communication with your healthcare team, and any referrals needed. *Any visit that follows a prenatal visit (same infant) is considered an Established Patient Visit/Follow up.
New Patient Visit --(175$) 1 to 1.5 hours--A New Patient visit includes a detailed medical history for the mother and infant, a limited physical exam of the mother and infant, a written care plan, communication with your healthcare team, and any referrals needed.
Established Patient Visit/Follow up--(125$) 1 hour--An Established Patient Visit/Follow reassessment of goals and concerns, a limited physical exam of mother and infant as needed, a written care plan, communication with your healthcare team, and any referrals needed.
FAQ Billing:
A cancellation fee will be billed for all visits that are not canceled within 24 hours of the appointment time.
A travel fee will be added to all visits that require over 30 minutes of travel time. This fee will be discussed with you before scheduling an appointment and will be based on the location of your visit.
Payment is expected at the time of your appointment. Cash, Credit, HSA, and Checks are accepted.
If the visit is not covered by your insurance I am glad to provide you with a Superbill (a detailed receipt with insurance billing and diagnosis codes) for you to submit to your insurance to seek reimbursement. There is no guarantee that your insurance will reimburse you but some do.
