This form should be submitted along with the DS-11 Application for a Passport, and brought with you when you go in person to apply for a new passport if your previous passport has been lost or stolen.

The payment form on Pay.gov is encrypted to ensure the security of your personal information. Pay.gov is a secure portal, used by more 40 U.S. government agencies, to collect forms and payments. Pay.gov uses 128-bit SSL encryption to protect your transaction.

The form may be completed by typing the required information into the online Passport Application Wizard or into the boxes on the electronic form (PDF 1.15 MB). The application should then be printed on white A4 or letter-size paper, with each page on a separate sheet of paper.

You can fill out the form using the online form filler of the Department of State website ( ) or print it out and fill it out by hand using a black pen. If you fill out the form electronically, at the end of the questionnaire there is an option to generate the form as a PDF file. Once you have generated the PDF file, you must print it (please do not print on double sided paper).

After you receive your renewed passport, if you change your name or find an error, you will need to get an updated one to reflect the change. The process, cost, and forms you need depend on how long you have had your current passport. Follow the steps from the State Department to change your name or correct your passport.

Applications to renew a damaged passport via mail may be accepted at our discretion if all the identifying information is still legible. If the photograph or identifying information is illegible, or the passport is significantly damaged, please refer to the information regarding Lost/Stolen/Damaged passports.

Step 5: SIGN, DATE, AND COMPLETE the passport renewal form DS-82. This form is completed online, not handwritten (print one-sided pages only, complete barcode appearing in upper left-hand corner). Please be sure to complete all fields and include your Spanish telephone number, Social Security number, and emergency contact. You will not be charged when you select the service fee on this form.

Starting April 18, 2022, U.S. citizens in Colombia and Venezuela who are eligible to use a DS-82 form will be able to renew their passports by mail. If you submit the wrong form, you will not receive a refund. You will be asked to submit the correct form and pay the correct fees.

If you are applying for a passport for the first time after age 16, or if you are unable to present a prior 10-year passport (e.g. your last passport was lost or stolen), you are not eligible to use Form DS-82. You must apply in person at the U.S. Embassy in Bogota or Consular Agency in Barranquilla with a DS-11 application form.

Send your completed and signed DS-82 form (printed single-sided), passport, 55 cm passport photo, online payment confirmation, name change document (if applicable), and return shipping waybill as detailed in the shipping instructions below.

If you DO NOT meet the above criteria, please make an appointment online. Please note that families with minors renewing passports with DS-11 forms MUST make appointments online through the scheduling system here. This application requires both parents and the minor child to be present at the interview.

U.S. citizens in Indonesia should replace a limited emergency U.S passport with a full-validity passport at the U.S. Embassy in Jakarta or U.S. Consulate General in Surabaya. The replacement will be done at no additional charge if you request it before the emergency passport expires. Please do not mail your application to a passport agency in the United States. Save time by completing and printing your passport application form before coming to the Embassy.

You are required to provide your SSN, if you have one, when you apply for or renew a U.S. passport. If you have not been issued a SSN, write zeros in box # 5 of the passport application form you are completing. To apply for a SSN please refer to the information here. If you have a number, but do not remember it, contact the regional office in Manila and request that they email your SSN to our ACS office in Jakarta or Surabaya.

If you do this, provide this person with a signed letter of authorization stating that you permit them to file a passport application on your behalf. You must sign the DS-82 form yourself; no proxy is allowed. Please note that if the passport official has any question about your eligibility for a U.S. passport, you will be required to appear in person.

Beginning October 1, 2023, you must generate your application form online, print it, and bring it with you. Blank application forms will no longer be available at the embassy, and we will no longer accept handwritten or typed passport application forms. The form must have a barcode printed in the top left corner of the page. If you do not bring the correct application form with a barcode as instructed, we will not accept your application and you will be advised to schedule another passport appointment.

A passport execution fee of $35 applies to U.S. passport applicants using the DS-11 form including: first-time applicants over age 16, children under age 16, applicants who reapply after reporting their previous passport lost or stolen, and applicants whose last U.S. passport was issued more than 15 years ago.

Customers applying with the DS-11 form at a U.S. embassy or consulate abroad pay two separate fees: an application fee to the U.S. Department of State and the execution fee to the Passport Acceptance Facility (such as a U.S. embassy or consulate abroad).

7. Provide 2 U.S. passport photos:

Your passport photo must be in color, 22 inches (5151 mm) in size, with a white background. It must have been taken within the last 6 months to reflect your current appearance, and in full-face view directly facing the camera.

Use the Online Passport Status System to check your application status if you applied by mail or in person. You may not be able to get a status update for 2 weeks after you apply or renew. During these weeks, your application and supporting documents are on their way to the Department of State.
