Parents

All scouts and hiking parents are required to complete and submit medical forms A, B and C annually, with form C requiring completion by a doctor. Link to BSA Medical Forms A, B & C. You may submit the original forms to Kumiko Sasamori at troop and patrol meetings. Although HIPAA rules do not apply to scout troops, please do not email any medical forms, as the BSA does not allow electronic sending or storage of medical records at this time.
